Answer Explanation

In the first question, the changes are being insisted to be made and are not yet made. Thus, we have to use 'be made' to denote that the the members want the changes to be made in future. 'are'/ 'is' denote present tense while 'being' denotes present continuous tense. 'be made' shows that changes must be done. Hence, option (4) is correct.
In second question, 'helped form' denotes that the company was formed with the help of Henry in past. We cannot use the name of person and 'he' together, thus option (3) is incorrect. 'helping' is present continuous tense, hence cannot be used to denote past events. Option(2) is the most appropriate.
